Ovarian cysts: which need watching and which need removing

Most ovarian cysts are functional — a normal part of ovulation that resolves on its own. A minority need more than observation.

- Simple cysts under 5cm are usually rescanned rather than removed
- Persistent cysts beyond two or three cycles need review
- Complex features on ultrasound prompt further assessment
- Large cysts carry a risk of twisting and are usually removed
Important
Sudden severe one-sided pelvic pain with vomiting can indicate ovarian torsion and is an emergency.

When should I see a doctor about ovarian cysts?
Book a consultation if symptoms persist, get worse, wake you at night, or come with bleeding, fever or unexplained weight loss. Anything that does not settle within a few weeks is worth having looked at.
Will I need surgery?
Often not. Many conditions settle with medication, physiotherapy or a change in daily habits. Surgery is discussed when simpler measures have been tried, or when the problem is structural.
