Lump near the anus: what it could be

Finding a lump near the anus is alarming, but most turn out to be one of a small number of benign causes. How quickly it appeared, and whether it hurts, narrows it down considerably.
The usual possibilities
- Thrombosed external pile — appears suddenly and is very painful
- Skin tag — painless, soft, often left over from an old pile
- Perianal abscess — hot, tender, may come with fever
- Prolapsed internal pile — appears on straining
- Pilonidal sinus — sits higher, in the cleft rather than at the anus
Important
A lump with fever or spreading redness suggests an abscess and should be seen the same day.

That pattern most often describes a thrombosed external pile — a clot inside an external pile. It is not dangerous but it is painful, and treatment within the first couple of days gives the fastest relief.
