Tonsillectomy in adults: who actually benefits

Adults are offered tonsillectomy based on how often infections occur and how disruptive they are, not on how large the tonsils look.
The usual thresholds
- Seven or more episodes in one year
- Five or more a year across two years
- Three or more a year across three years
- Recurrent abscess around the tonsil
- Obstructive sleep apnoea related to tonsil size
Important
Adult recovery is slower and more painful than in children, typically two weeks off work. That is normal, not a complication.

When should I see a doctor about tonsil infections?
Book a consultation if symptoms persist, get worse, wake you at night, or come with bleeding, fever or unexplained weight loss. Anything that does not settle within a few weeks is worth having looked at.
Will I need surgery?
Often not. Many conditions settle with medication, physiotherapy or a change in daily habits. Surgery is discussed when simpler measures have been tried, or when the problem is structural.
